The following forms can be viewed by downloading the PDF below.
You will be required to sign these documents prior to you stay with us. You can sign these documents at check-in or online through our customer portal. If you are a new customer simply click the "Book Online Today" button to get started.
Once you are in our system you will be able to sign these forms by simply selecting Agreements from the drop down menu of the "My Account" tab.
Boarding Agreement (PDF)
This form specifies the pet owners' and our (Montage) responsibilities.
Vet Care Directive (PDF)
This form outlines our procedures in the event that your pet requires veterinary attention.
